My first 3 years of my job, I prepared a 2 pager of my accomplishments for my boss.

It sounds a little self-serving but I think was helpful for him and her to remind them of what I accomplished.

I both described all the projects I worked on and results. But also put in quotations in appendix from emails where people said I did an awesome job, etc.
