How is government really doing?

I have put together a survey to determine how citizens feel about the efforts of their local and federal governments.The focus is on Government 2.0 communication and collaboration and can be found here:

In order to gain as large a set of responses as possible, on as global a scale as possible, I am asking if you can take 5 minutes to answer the survey yourself and then share it with as many people on as many networks as you can. I am planning on closing out this survey in the middle of August.
